General Information

SHS delivers school health services through a team approach with registered nurses and clinic assistants who work in the school-based clinics. Student Health Services serves as a resource for all school health related issues.

All prescription medications require you to complete a Fulton County Medication Authorization Form SHS-1. A doctor's signature is required on this form for all prescription medications, such as inhalers, antibiotics, and epi-pens. All prescription medications must be in their original containers with the pharmacy labels intact. It is your responsibility to continue supplying the clinic with your child's prescription medications.

Non-prescription medications also require you to complete an authorization form SHS-1, but no physician's signature is required, only that of a parent or guardian. Examples of non-prescription medications include topical creams, cough drops, cough syrup, Tums, Benadryl, Tylenol, Advil, and Midol. All non-prescription medications must be in their original containers with the child's name written on the container. They should have an expiration date later than 5/15 of the current school year.

If you have a student who has asthma or other health conditions that require self-administration of medication, you must obtain a form Authorization For Students To Carry A Prescription Inhaler, EpiPen, Insulin, or other Approved Medication. This form, including written permission from your doctor and your signature must be completed before the school can allow your child to carry medication.

Hand written notes, faxes, expired medications, or medication not in its original container will not be accepted for prescription or non-prescription medications in accordance with Fulton County Board of Education Guidelines.

If your child has any medical condition of which you would like us to be aware that may require an emergency medical plan, please contact us. Examples are asthma, epilepsy, diabetes, and allergies to foods, medications, or insect bites.

Parents, guardians, or individuals on emergency contact lists will be notified by phone if the child sustains a significant injury on school grounds. Please provide the school with all home, business, and emergency phone numbers, as well as cell-phone and pager numbers. This will ensure that we are able to contact you in a timely manner.

If your child is diagnosed with any communicable disease (flu, strep throat, head lice, chicken pox, etc.) please inform the Clinic. When appropriate, we inform parents so they may watch for specific symptoms at home, lessening the chance of the illness spreading further in our school.
